6. Liability Waiver & Assumption of Risk

1. LIABILITY WAIVER AND ASSUMPTION OF RISK I confirm that I have been informed of the risks of scuba diving. I understand that diving with compressed gases carries risks such as decompression sickness or embolism. I release Scuba Quest Phuket, its staff and the vessel owners from any liability for accidents or injuries, unless caused by gross negligence. I accept full responsibility for my own safety and belongings. 2. MEDICAL SELF-DECLARATION I confirm that my answers in the medical questionnaire are truthful. If I answered YES to any question, I hold a valid medical certificate. In the event of injury I authorise Scuba Quest Phuket to administer first aid and arrange transport to a medical facility. 3. EQUIPMENT & PERSONAL PROPERTY I accept responsibility for the rental equipment provided to me. In the event of loss or damage caused by gross negligence, I agree to pay the replacement value. Scuba Quest Phuket accepts no liability for the loss or damage of personal belongings on board. 4. Privacy Policy & Terms & Conditions I agree that my data may be stored for the purposes of the relevant authorities (immigration/tourism authority).
